Surgical Protocols

Post-Surgical Timeline:

Repair of Achilles Tendon Rupture:

  • Week 1-2: Non-weight bearing in a splint with strict elevation
  • Week 3-6: CAM Walker boot with heel lift and partial weight bearing; active dorsiflexion ROM to neutral position
  • Week 7-12: Begin formal physical therapy and continue CAM Walker boot
  • Month 3-6: Transition out of boot into a sneaker; Return to many normal activities of daily living; Continued physical therapy
  • Month 6-12: Will continue to see improvement and gradual return to sports

Surgical Repair of Achilles tendinosis:

  • Week 1-2: Non-weight bearing in a splint with strict elevation
  • Week 3-4: NWB in a cast
  • Week 5-12: CAM Walker boot with partial weight bearing; begin formal physical therapy
  • Month 3-6: Transition out of boot into a sneaker; Return to many normal activities of daily living; Continued physical therapy
  • Month 6-12: Will continue to see improvement and gradual return to sports

Bunion Repair:

  • Week 1-2: Non-weight bearing in a splint with strict elevation
  • Week 3-4: Darco shoe with partial weight bearing and passive ROM of great toe 100 times per day
  • Week 5-8: Begin formal physical therapy and transition into a sneaker
  • Month 2-3: Return to many normal activities of daily living; Continued physical therapy
  • Month 4-12: Will continue to see improvement and gradual return to sports

Hallux Rigidus Repair:

  • Week 1-2: Non-weight bearing in a splint with strict elevation
  • Week 3-4: Darco shoe with partial weight bearing and passive ROM of great toe 100 times per day
  • Week 5-8: Begin formal physical therapy and transition into a sneaker
  • Month 2-3: Return to many normal activities of daily living; Continued physical therapy
  • Month 4-12: Will continue to see improvement and gradual return to sports

ORIF Ankle Fracture/Foot Fracture:

  • Week 1-2: Non-weight bearing in a splint with strict elevation
  • Week 3-6: NWB in a cast
  • Week 7-12: CAM Walker boot with partial weight bearing and begin formal physical therapy
  • Month 3-5: Transition out of boot into a sneaker; Return to many normal activities of daily living; Continued physical therapy
  • Month 6-12: Will continue to see improvement and gradual return to sports

Midfoot Fusion:

  • Week 1-2: Non-weight bearing in a splint with strict elevation
  • Week 3-6: Non-weight bearing in a cast
  • Week 7-12: CAM Walker boot with partial weight bearing and begin formal physical therapy
  • Month 3-5: Transition out of boot into a sneaker; Return to many normal activities of daily living; Continued physical therapy
  • Month 6-12: Will continue to see improvement and gradual return to sports

Ankle Stabilization:

  • Week 1-2: Non-weight bearing in a splint with strict elevation
  • Week 3-6: Non-weight bearing in a cast
  • Week 7-12: CAM Walker boot with partial weight bearing and begin formal physical therapy
  • Month 3-5: Transition out of boot into a sneaker; Return to many normal activities of daily living; Continued physical therapy
  • Month 6-12: Will continue to see improvement and gradual return to sports

Ankle Stabilization with microfracture:

  • Week 1-2: Non-weight bearing in a splint with strict elevation
  • Week 3-4: Non-weight bearing in a cast
  • Week 5-6: Non-weight bearing in a CAM walking boot with active ankle ROM and begin formal physical therapy
  • Week 7-12: Partial weight bearing in CAM walking boot
  • Month 3-5: Transition out of boot into a sneaker; Return to many normal activities of daily living
  • Month 6-12: Close to maximal medical improvement

Ankle Arthroscopy and Debridement:

  • Week 1-2: Non-weight bearing in a splint with strict elevation
  • Week 3-6: CAM walking boot and begin formal physical therapy
  • Mont 2-3: Transition out of boot into a sneaker; Return to many normal activities of daily living; Continued physical therapy
  • Month 4-12: Will continue to see improvement and gradual return to sports

Ankle Arthroscopy with Microfracture:

  • Week 1-2: Non-weight bearing in a splint with strict elevation
  • Week 3-8: Partial weight bearing in CAM walking boot and begin formal physical therapy
  • Mont 2-3: Transition out of boot into a sneaker; Return to many normal activities of daily living; Continued physical therapy
  • Month 4-12: Will continue to see improvement and gradual return to sports
